Assessing physical rehabilitation programs to develop cross-platform research-based approaches.
Problem: Most hospitals and healthcare systems provide some traditional rehabilitation services. New pressures on whole system efficiency and outcomes create both a risk and an opportunity for rehabilitation to lead change through integration of services from the intensive care unit to the home health agency.
Solution: Starting with strong traditional inpatient and outpatient rehabilitation services, we develop intelligent healthcare systems that improve quality and reduce cost of care by using rehabilitation teams to bridge gaps and change culture.
Approach: We assess current strengths, weaknesses, and processes. Then we design across platforms and specialties – from prevention and pre-habilitation to discharge planning, coordination with inpatient rehabilitation facilities, post-acute facilities, home health agencies and outpatient services. Our focus is on the details: Space design, equipment, staffing, financials, protocols, marketing and payer relations.
Examples of our work:
- Designing a rehabilitation system for Aurora Health Care, the largest healthcare organization in Wisconsin.
- Providing a system-wide analysis of the pain management needs for CentraCare in Minnesota.
- Training staff and designing a facility for YuhoCare’s 300 bed rehabilitation hospital in Beijing.
- Developing Accountable Care clinical strategies for Mary Free Bed Rehabilitation Hospital (now the nation’s fifth-largest rehabilitation system) within the hospital and with other partners that strengthened negotiations with payers and improved care.
